We use a Digital Check-In that begins with a simple text message or email, including a secure link to begin the check-in process. No app download or portal account needed. Patients can complete as much—or as little—as they want before their appointment. Whatever they complete will be saved to their record, and they can simply pick up where they left off when they arrive. No lost work. No duplicate entry. You will be asked to complete Demographics, Insurance and Medical History Information, as well as read and consent to our Office Financial & Billing Policy, and Consent to Treat forms.
Digital Check-In Prior to Appointment
- If you have provided an email or agreed to receiving text messages from our office, you will receive a simple message invitation from Southern Tier Podiatry to Check-In for your appointment. It will walk you through steps to complete your Demographics, Insurance, Medical History, Office Billing Policy Agreement and Consent to Treat. This can be done on a phone, tablet or computer prior to your appointment.
- If you started, but did not complete, the Digital Check-In, you will be asked to complete this in our office, using one of our ipads.
- Our office staff is here to assist you in using the ipads, or if computers are just not your thing.
- Remember to still bring a photo ID, and your medical insurance card(s), as well as a referral, if your insurance plan requires it, to your appointment. If you are unsure if a referral is required, please call your insurance carrier to find out. The referral is then obtained from your Primary Care Physician.
We have also made available our office’s Privacy Practices / HIPAA Form. Please review, and if any questions, please ask. This is available for your information. You do not have to print this form out prior to your 1st appointment with us.
